These three sample photography release forms are intended for use when consent is needed for photography or video (such as when hosting an event). Note: Please ensure each form is printed with the description included, so that all signatories have the opportunity to review the disclaimer prior to signing.

Sample Photo Release – Individual Signature: This is a sample photo release form that can be used whenever individual consent is needed for the use of photographs or video. Each person should sign a separate copy to provide their authorization.

Sample Photo Release – Group: This is a sample photo release intended for use when hosting an event with multiple participants. To streamline documentation, a single form may be signed by multiple individuals. 

Comprehensive Sample Photo Release – Individual Signature: This is a sample photo release form that can be used whenever individual consent is needed for the use of photographs or video. This release includes the option for participants to revoke their consent at any time in the future, provided such revocation is submitted in writing. Please note that revocation cannot affect materials already published or distributed prior to the date of revocation.
